About The Job:

The Social Services Technician collaborates with other members of the multidisciplinary team in caring for the psychosocial, financial, emotional, environmental factors as they relate to patient health needs; acts in concert with the social services, nursing, medical, and other support staff in providing assistance and supportive measures to patients and families for care coordination and care management – specifically focused on connecting patients with community resources and helping patients to be successful in removing socio-economical barriers; participates in ongoing continuous quality improvement processes to improve patient care.

What we are looking for:

Education:

Bachelor of Science or Arts degree with a major in social services, psychology, sociology, human relations, health related field required.

Experience:

Previous experience working in connecting individuals to community resources is required.

Special Skills, Competencies, and Aptitudes:

Ability to work on multiple projects at one time and flexibility to meet department needs.

Self-motivated, detail oriented individual able to work with minimum supervision.

Completes the following mandatory courses (check all that apply):

Must provide proof of Basic Life Support (BLS Healthcare Provider, American Heart Association; or American Red Cross, Professional Rescuer-LIVE class only) via current card prior to employment or within 60 days of hire and maintains current status.

Completes Mandatory Reporting: Child and Dependent Adult Abuse requirement within 6 months of hire and renews this requirement every 5 years.
